New Patients

Step one, call and schedule

Call (772) 678-0011 and say you are a new patient. Tell them which of these you need, because it sets the visit type and how much time is held:

• An annual physical
• Transfer of care for a condition you already manage
• A specific problem you want looked at
• A school or sports physical
• Something urgent today. Say so, and we will find the soonest appointment

Step two, gather what you are bringing

Required

• Photo identification
• A method of payment. We accept cash and card

Bring whatever you have

• All current medications in their original bottles, including over the counter, vitamins, and supplements. Bottles beat a list, the doses and prescribers are printed on them
• Immunization record, especially for children and teens
• Recent lab and imaging reports, the actual reports not just the dates
• Names and contact details for any specialists you see
• Hospital or emergency records from the past year
• A written list of your questions. The most useful thing you can bring
• For a minor, a parent or legal guardian for consent

Step three, start your records transfer

You do not need your chart before the first visit, but starting early makes the second one much better.

• Call your previous practice and ask for their records release form
• Sign it. They cannot release anything without your signature, it is a HIPAA rule

• Allow a few weeks. Requests between practices are slow
• If records never arrive, come anyway. A careful history and fresh labs rebuild most of what matters

What happens at your first visit

1. Intake. Health history, medications, allergies, insurance, privacy acknowledgment, consent to treat. Arrive 10 to 15 minutes early, or complete it in advance through the portal.
2. Vitals. Blood pressure, heart rate, temperature, oxygen saturation, height, weight.
3. The visit. Your provider reviews your history in depth, examines you, and works out what needs attention first. New patient visits are scheduled with more time than routine ones.
4. Orders and next steps. Labs, imaging, referrals, prescriptions, and a follow up if one is needed.
5. Results. Your provider reviews your labs and explains what they mean, rather than posting numbers without context.

One honest note

If you have not seen a provider in years, or several things are going on, one visit may not resolve everything. That is normal.

Your provider will prioritize with you: what is urgent, what needs a workup, and what can wait. Eight problems in one appointment produces eight shallow answers. Two handled properly, with the rest scheduled, is better medicine.

Tell the front desk your list when you book so they can hold enough time.

Payment

Seaside Family Wellness participates with some insurance plans, but not all. Participating plans change frequently, so call (772) 678-0011 before your visit to confirm whether we currently participate with your specific plan. Self-pay options are available, and labs and diagnostics can sometimes be billed through your insurance when medically appropriate.

Bring your insurance card. We will check whether we currently participate with your plan, and the card may also be usable for labs or diagnostics when medically appropriate. Having it on file saves time later.
