Call 911 or go to the nearest emergency department for chest pain, trouble breathing, sudden weakness or numbness on one side, trouble speaking, a sudden severe headache, uncontrolled bleeding, a seizure, fainting, or a severe allergic reaction with swelling or throat tightness. Do not wait for an office appointment. If you are in a mental health crisis or thinking about harming yourself, call or text 988 to reach the Suicide and Crisis Lifeline, any time of day.

We ask for at least 24 business hours notice to cancel or reschedule, so the appointment time can be offered to another patient. Without adequate notice, a missed appointment or late cancellation may be charged: $25 for an established patient visit, $50 for a new patient visit, and $50 for an annual physical or extended visit. These fees are not billable to insurance.

Seaside Family Wellness participates with some insurance plans, but not all. Participating plans change frequently, so call (772) 678-0011 before your visit to confirm whether we currently participate with your specific plan. Self pay options are available, and labs and diagnostics can sometimes be billed through your insurance when medically appropriate. We accept cash and card, and self pay prices are published in full.

Photo ID, insurance card, a current list of medications and supplements, and any records or a summary from a previous provider. Arrive 10 to 15 minutes early so there is time to finish check in.
